How much muscle do you lose as you age?

Short answer: about 3–8% of your muscle mass per decade after age 30, and the rate accelerates after 60. Left alone, that adds up to clinically low muscle — sarcopenia — in 10–27% of people, depending on the definition. But the loss is not a one-way street: frail nursing-home residents around 90 gained 174% in strength and 9% in thigh muscle area in eight weeks of lifting, and every 5 kg of grip strength you keep is associated with 16% lower risk of death. Inactivity drives most of the decline; training reverses a surprising amount of it.

How fast do you lose muscle after 30?

Three to eight percent a decade. The review by Volpi, Nazemi and Fujita states that muscle mass decreases approximately 3–8% per decade after the age of 30 and that the rate of decline is even higher after the age of 60 (Volpi et al., Current Opinion in Clinical Nutrition and Metabolic Care, 2004). Westcott's review applies the same 3–8% figure specifically to inactive adults, and notes it comes with a falling resting metabolic rate and rising fat mass (Westcott, Current Sports Medicine Reports, 2012). The word doing the work in both is inactive: the numbers describe what happens by default, not what has to happen.

How common is sarcopenia?

Between one in ten and one in four. Across 151 studies and 692,056 people (mean age 68.5), the prevalence of sarcopenia — clinically low muscle mass and strength — ranged from 10% to 27% depending on the diagnostic criteria, from 8% to 36% in people under 60 and 10% to 27% in those 60 and over (Petermann-Rocha et al., Journal of Cachexia, Sarcopenia and Muscle, 2022). The spread reflects how many definitions exist, not uncertainty about the trend.

Why does muscle loss matter?

Because strength predicts survival. In the PURE study of 139,691 adults across 17 countries, each 5 kg reduction in grip strength was associated with a 16% higher risk of death from any cause and 17% higher cardiovascular mortality, and grip strength was a stronger predictor of mortality than systolic blood pressure (Leong et al., The Lancet, 2015). Adults who do any resistance training have 15% lower all-cause mortality than those who do none (Shailendra et al., American Journal of Preventive Medicine, 2022), and muscle-strengthening activity is linked to 10–17% lower risk of death, cardiovascular disease, cancer and diabetes (Momma et al., British Journal of Sports Medicine, 2022).

Can you rebuild muscle after 50, 60 or 70?

Yes, at every age that has been tested. The most striking trial took ten frail, institutionalised volunteers aged 90 ± 1 through eight weeks of high-intensity resistance training: strength gains averaged 174%, mid-thigh muscle area increased 9.0% and tandem walking speed improved 48% (Fiatarone et al., JAMA, 1990). In postmenopausal women with low bone mass, eight months of twice-weekly, 30-minute heavy lifting raised lumbar-spine bone density 2.9% while the comparison group lost 1.2%, with only one minor adverse event in 101 participants (Watson et al., Journal of Bone and Mineral Research, 2018). For adults generally, ten weeks of resistance training adds about 1.4 kg of lean weight and raises resting metabolic rate 7% (Westcott, 2012).

How much strength training do you need to keep your muscle?

Two days a week is the guideline; 30–60 minutes a week is where the survival benefit peaks. The CDC recommends muscle-strengthening activity on at least two days a week (CDC); the cohort analyses put the largest mortality reduction at roughly 30–60 minutes a week (Momma et al., 2022). Most older adults are nowhere near it: among men meeting both federal activity guidelines, the share fell from 41.3% at ages 18–34 to 15.3% at 65 and over, and among women from 28.7% to 10.8% (CDC/NCHS, 2022) — the age groups losing muscle fastest are the ones lifting least.
